A new report has called for GP surgeries to play a key role in the detection, prevention and treatment of malnutrition.
It follows findings that nearly 31% of people transferred from one hospital to another and more than a third of patients sent from hospitals to care homes were at risk of malnutrition, according to a report by the British Association for Parenteral and Enteral Nutrition (Bapen).
Christine Russell, co-project lead for the study, said: "Healthcare professionals
The Bapen report said results of screenings should be linked to care plans, which may vary according to local resources and policy.
And it said "consistent strategies to detect, prevent and treat malnutrition should be in place in care settings, including the community, where most malnutrition occurs." Patients should also have access to nutrition advice and support teams, it said.
The report's authors said more needed to be done to tackle the problem across all settings.
Patrick South, head of public affairs of Age Concern, said: "The government's plans to tackle malnutrition must look beyond the hospital setting and also address malnutrition in the community."
